Departing captain John Terry made an emotional farewell to the Stamford Bridge faithful when he was substituted on 26 minutes before Eden Hazard put Chelsea ahead in the second half with a powerful shot into the far corner.
Pedro's close-range header put the Blues 3-1 ahead before Michy Batshuayi added two late goals as Chelsea began the trophy celebrations in style.
Chelsea finish the campaign on 93 points, seven clear of Tottenham Hotspur, and have become the first team to claim 30 wins in a Premier League season.
