Fornals put West Ham ahead on 30 minutes when he followed up Alex McCarthy’s parry of Jarrod Bowen’s shot.
Vladimir Coufal teed up Fornals to score again three minutes later with a low finish.
Lukasz Fabianksi saved from Minamino and substitute Nathan Tella before West Ham wrapped up the win on 86 minutes through Declan Rice’s near-post finish from Fornals’ pass.
West Ham secured a first top-six finish in 22 years with 65 points. Southampton drop one place to 15th, with 43 points.
